PURPOSE
To provide several one-year scholarships of $1,000 each. The
awards will be based on academic excellence, leadership,
test scores and extra curricular activities. Students
receiving scholarships from other sources may not be
eligible for a Center Grove Scholarship Foundation award.
ELIGIBILITY CRITERIA
To be eligible to apply for funds through the Foundation
students must:

Be a Center Grove
High School, Greenwood, IN class of 2012 senior with a 2.67 B- or
above GPA |
|
|
Be planning to
enroll as a full-time student in a 1, 2 or 4-year
post secondary program at a college, university,
vocational or technical school for 2012-2013. |

Students who
anticipate receiving scholarship aid of over $5,000
from other sources may not be eligible for the
Phonothon Funded Scholarships. However, ALL students may
apply for and be eligible for the Donor Sponsored
Scholarships listed in this application.

It is important
that the entire application be completed; however,
some questions may not apply. If this is the case,
simply put NA as the answer. Reminder: Incomplete
applications will not be considered. |

Awards will be
announced on Honors Night and recipients will not be
notified ahead of time. IMPORTANT: It is expected
that all students who have applied for a CGSF
scholarship will be in attendance at the awards
ceremony to possibly receive an award and to be
recognized by their peers. This is an honor! |

Actual scholarship
funding will be sent directly to the Office of
Student Financial Aid at the chosen institution.
Funding is disbursed prior to the fall and spring
terms. |
Submission deadline is
Friday, March 16, 2012, TO THE GUIDANCE OFFICE at the
close of the school day. Submittal after this date may not
receive full consideration. DONT DELAY IN THE COMPLETION OF
THIS APPLICATION!
Once you have completed the online
application, you must print and sign the
transcript request
form. Please return this completed CGSF
Transcript Request Form to Mrs. Duffey in the guidance
office prior to the March 16th deadline at 3:00pm. If you have

Grove Scholarship Foundation, it is appropriate that
you send a thank you note to the sponsor or family
or directly to the Center Grove Scholarship
Foundation. Names and addresses will be provided to
you for this purpose. |
